Instruction for Completing the Black Farmers Association Membership Application and the Questionnaire

1. Membership Application: If filing for yourself, print your name on the first line
2. Line 2 (On behalf), if for yourself write the word self and proceed to the line for address etc. and sign form
3. If you are filing on behalf of someone else: Put your name of line 1 and the name of the person who you are filing on behalf on line 2
4. 3rd line ( Your relationship to that person) write relationship such as daughter, son, mother, brother, father etc.
5. Sign at the bottom where it reads Signature and date.
Questionnaire And Petition form
1. Fill out the first page
2. Skip section A,B&C.
3. Complete Section D. Answer to the best of your knowledge, your answers will not disqualify you. However on question number 37 answer yes and 38 answer no.
If you are filing on behalf of someone else they would have had to have been alive during the period from Jan 1, 1981 thru Dec. 31, 1996 or after 1996
