Titles and Positions - Who Appoints them?

Within the Black Church there appears to be a preoccupation with titles. Today it seems you are not legitimate unless you have Apostle, Prophet, or Bishop in front of your name. Why are there so many Bishops and Apostles? Do the size of the church have anything to do with the titles or these appointments.

The bible says that He gave some to be Apostles, Prophets, Evangelists, Pastors, and Teachers. I know some peple that won't invite you to their church unless your an Apostle, Prophet or Bishop. My mentor has over 7,000 members in his church and he want to be known simple as a Pastor.

Have these titles been overused? What constitute from a biblical standpoint someone walking in these offices.?

I am extremely curious.
